Nuvi Global Corporation of Rancho Cucamonga California is recalling StemVitae 30oz liquid multivitamin because it contains undeclared milk and soy lecithin. People who have an allergy or severe sensitivity to milk or soy run the risk of serious or life-threatening allergic reaction if they consume this product.

The product is in a tall white plastic bottle with white lid. The brand is Nuvi Global with a white and orange label that has the name StemVitae. The label contains an image of various fruits and vegetables. The lot number is 15556 and can be found printed in black ink underneath the bottle.

StemVitae was distributed in the following states:

Idaho
Nebraska
Oklahoma
Arkansas
Illinois
Nevada
South Carolina
California
Indiana
New Jersey
Texas
Colorado
Iowa
New Mexico

No illnesses have been reported to date.

The recall was initiated after it was discovered by the FDA that the ingredient whey protein was not declared on the label. The whey protein ingredient contains the allergens of milk and soy lecithin.

Exceptional Health Products of Tulsa Oklahoma is recalling Angel Wings™ – Daily Multi 120 Capsules because this product contains undeclared SOY and MILK allergens. People who have an allergy or severe sensitivity to Soy or Milk allergens run the risk of serious or life-threatening allergic reaction if they consume this product.

The following product was distributed nationwide to consumers through mailorder.

Product Name: Angel Wings™ – Daily Multi 120 Capsules

Lot Numbers: 14656, 14657, 15109, 15489, 15488, 15616, and 16007

This voluntary recall has been initiated because this product contains undeclared SOY and MILK allergens. People who have an allergy or severe sensitivity to Soy or Milk allergens run the risk of serious or life-threatening allergic reaction if they consume this product.

No illnesses have been reported to date. This was brought to our attention by the Food and Drug Administration that found inconsistencies with our supplier’s allergen statement on the Amino Acid ingredients used in this formulation.

The recalled lot numbers began shipping nationwide on December 23, 2014. If you find the recalled lot numbers on the bottom of any bottle you have inventory you may return it to the address at the bottom for a full refund or replacement. The Zuma Supreme lot numbers began shipping nationwide on December 23, 2014 and the Life’s Qik Fix on February 8, 2016. If you find the recalled lot numbers on the bottom of any bottle you have in inventory you may return it to the address at the bottom for a full refund or replacement. CVS Pharmacy Announces Voluntary Recall of Gold Emblem Abound Organic Spiced Herbal Tea Due to Possible Health Risk – 4/26/16

CVS Pharmacy® announced today that it has voluntarily recalled select cases of Gold Emblem Abound™ Organic Spiced Herbal Tea following notification from the manufacturer that the product is potentially contaminated with Salmonella. An ingredient produced by the manufacturer’s raw material supplier tested positive for Salmonella in another company’s product. CVS Pharmacy’s Gold Emblem product also contains this ingredient.

The product is labeled “Gold Emblem Abound Organic Spiced Herbal Tea 1.41 oz” and was packed in 1.4 oz cartons. The recalled product has a single best by date of 18 Mar 2018 with a UPC code 0 50428 541043. No other best by dates are affected. The product was available at CVS Pharmacy stores nationwide.

Salmonella is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

Approximately 200 units of the recalled product were sold prior to this recall. The Company is not aware of any reported cases of illness to date related to this product and it has issued this voluntary recall as a matter of precaution.

CVS Pharmacy has removed the affected products from its stores and the manufacturer has notified the U.S. Food and Drug Administration. A “do not sell” block has also been placed in the Company’s store register system to prevent further sale of these recalled items.

Any customer who purchased the recalled product should immediately discontinue use and return the item to CVS Pharmacy for a refund. For additional information, please contact the Company at 1-800-SHOP-CVS Monday – Friday 8:30 AM – 7:00 PM ET.

Back to Nature Expands Voluntary Recall for Limited Number of Classic Creme Cookies Due to Undeclared Milk – 4/21/16

Naples, FL – Back to Nature Foods, LLC is expanding its voluntary recall to include four (4) additional lots of its Classic Creme cookies, 12 oz packages, because they may contain undeclared milk, not listed as an ingredient on the label. Persons who have an allergy or severe sensitivity to milk run the risk of a serious or life-threatening allergic reaction if they consume this product.

The affected product was distributed to retail stores nationwide. In addition to the previously announced affected packages that were marked with a unit UPC# 19898-01103 and with best by dates of 10 SEP 16 and 16 SEP 16 labeled on top of the package, Classic Creme cookies packages marked with a unit UPC# 19898-01103 and with best by dates of 13 AUG 16 and 08 OCT 16 and 21 OCT 16 and 22 OCT 16 labeled on top of the package were also affected.

Consumers who have purchased the Back to Nature Classic Creme cookies with the UPC and dates noted, and have an allergy to milk, should destroy the product they have or are urged to return to the place of purchase for product replacement or refund. No other best by dates are affected.

Perfect Fit Meals Recalls Poultry Products Due to Misbranding and Undeclared Allergen

The Turkey Enchilada items were produced on March 28, 2016. The Lemon Pepper Chicken items were produced on March 25, 2016. The following products are subject to recall:

  • 10.4-oz. sealed tray packages with a label that says “Perfect Fit Meals Texas Chicken Ranch Casserole” with a use-by date of 4/24/2016, but may contain “Perfect Fit Meals Lemon Pepper Chicken.”
  • 11-oz sealed tray packages with a label that says “Perfect Fit Meals Chipotle Chicken Sausage” with a use-by date of 5/2/2016, but may contain “Perfect Fit Meals Turkey Enchilada.”

The products subject to recall bear establishment number “P-827” inside the USDA mark of inspection on the sealed tray packages. These items were shipped to retail locations nationwide.

The problem was discovered after the firm was notified by a retailer that the products were incorrectly labeled.

Anyone concerned about an injury or illness should contact a healthcare provider.

Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

Milky Way International Trading Corp. Issues Voluntary Nationwide Recall of Nice! Peach Slices and Nice! Mixed Fruits in 8-ounce Glass Jars Due to Possible Glass in Products – 4/19/16

Milky Way International Trading Corp. is initiating a voluntary recall of Nice! Peach Slices and Nice! Mixed Fruit in 8-ounce glass jars out of an abundance of caution due to potential glass in the product. The affected products were distributed to Walgreens stores nationwide and display the lots below. Consumers could potentially be cut or injured if ingested. To date there has been one complaint on the peaches and no complaints on the mixed fruits.

The product has been removed from shelves in Walgreens stores.

The voluntary recall is limited to specific production codes as follows. The lot codes are located at the neck of the glass jars.

Reser’s Fine Foods, Inc. Recalls Refrigerated Salads Because of Possible Health Risk Due to Onion Supplier Recall – 4/13/16

Reser’s Fine Foods, Inc. of Beaverton, Oregon is recalling nineteen refrigerated salad items due to notification from one of our ingredient suppliers that Listeria monocytogenes may be present in one lot of onions that was used in the manufacture of these salads. Listeria monocytogenes is an organism which can cause serious and sometime fatal infections in young children, frail or elderly people and individuals with weakened immune systems. Healthy people may suffer only short term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea. Listeria infection can cause miscarriages and stillbirths among pregnant women.

The recalled refrigerated salads were distributed to retailers and distribution centers in the following states: AL, AR, AZ, CA, CO, HI, ID, IL, IN, KS, KY, MI, MN, MO, MT, NC, NE, NM, NV, OH, OK, OR, PA, SD, TN, TX, UT, WA, WY, and British Columbia.

The retail product is sold in plastic containers and is marked with a Use By Date and the #10 at the end of the line of printing (see photo below).

No illnesses have been reported to date.

The recall was the result of a notification from one of our onion suppliers that the bacteria may be present in one lot of onions used in the manufacture of these refrigerated salads. No other items were manufactured using this lot of onions.

Consumers are urged not to consume these products. Consumers who purchased these products may take them back to the store for a refund or discard them.

Pilgrim’s Pride Corp. Recalls Poultry Products Due To Possible Foreign Matter Contamination

WASHINGTON, April 7, 2016 – Pilgrim’s Pride Corp., a Waco, Texas establishment, is recalling approximately 40,780 pounds of fully cooked chicken nugget products that may be contaminated with extraneous plastic materials, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

The fully cooked chicken nugget products were produced on Oct. 5, 2015. The following products are subject to recall:

20-lb. cardboard boxes containing two, 10-lb. clear plastic bags of fully cooked chicken nuggets labeled as “GOLD KIST FARMS Fully Cooked Whole Grain Popcorn Style Chicken Patty Fritters.”

The products subject to recall bear establishment number “EST. P-20728” inside the USDA mark of inspection and include package codes 5278105021, 5278105022, 5278105023, 5278105000, and 5278105001. These items were shipped for institutional use to Arizona, California, Colorado, Florida, Georgia, Kansas, Kentucky, North Carolina, Nebraska, Oklahoma, Tennessee, Texas, and Utah.

The problem was discovered after the firm received several consumer complaints regarding plastic contamination of the chicken nuggets.  The firm notified FSIS personnel of the issue on April 6, 2016.

There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

Ukrop’s Homestyle Foods Recalls Ready-To-Eat Turkey, Pork, And Beef Products Due To Possible Foreign Matter Contamination – 4/8/16

WASHINGTON, April 06, 2016 – Ukrop’s Homestyle Foods, a Richmond, Va. establishment, is recalling approximately 2,881 pounds of turkey, pork, and beef  products that may be contaminated with extraneous materials, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

The ready-to-eat turkey, pork, and beef items were packed on April 1 and 2, 2016, and exhibit sell-by dates of April 4 and 5, 2016.  The following products are subject to recall: [Labels (PDF Only)]

  • 185.02-lb. of individually packaged 9.25 OZ “ROASTED TURKEY & COLBY JACK PINWHEELS.”
  • 161.82-lb. of individually packaged 9.25 OZ “BLACK FOREST HAM & PROVOLONE PINWHEELS.”
  • 153.70-lb. of individually packaged 9.25 OZ “ANGUS ROAST BEEF & CHEDDAR PINWHEELS.”
  • 821.76-lb. of 41OZ trays of “HONEY HAM & HONEY TURKEY PINWHEEL TRAY.”
  • 806.40-lb. of 41OZ trays of “HONEY TURKEY & HAVARTI PINWHEEL TRAY.”
  • 752.64-lb. of 41OZ trays of “HONEY HAM & SWISS PINWHEEL TRAY.”

The products subject to recall bear establishment numbers “EST. 19979” and “P-19979” inside the USDA mark of inspection. These items were distributed to retail locations in North Carolina, Virginia, and West Virginia.

The problem was discovered on April 4, 2016, when Ukrop’s Homestyle Foods received notice from their distributor of the recall involving Roland® Fire Roasted Red Pepper Strips.  (See FDA press release here.) The red pepper strips were recalled by Roland Foods, LLC on April 1, 2016, due to the possible presence of glass fragments in the product.

There have been no confirmed reports of adverse reactions due to consumption of these products. FSIS has received no additional reports of injury or illness from consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

Snyder’s-Lance, Inc. is initiating a voluntary recall for a limited amount of Emerald® 100 Calorie Pack Roasted & Salted Cashew Halves & Pieces product, distributed nationwide, due to the possible presence of small glass pieces. This voluntary recall covers only specific production codes of the following product:

Emerald® 100 Calorie Pack Roasted & Salted Cashew Halves & Pieces 7 Packs / 0.62oz

No injuries have been reported to date. We are recalling these products because they may contain small pieces of glass that could potentially cause injury. Although our investigation is ongoing, we believe the source of the glass to be the raw cashews received from one of our suppliers under a specific lot code.

We are taking this action out of an abundance of caution after receiving a consumer complaint.

Roland Foods, LLC, of New York, NY, in cooperation with the manufacturer in Peru, is initiating a voluntary recall of specific lots of Roland® Fire Roasted Red Pepper Strips due to the possible presence of glass fragments in the product, therefore posing a potential health hazard.

Roland® Fire Roasted Red Pepper Strips were distributed nationwide and to Canada to food distributors, food service customers, and super market chains for further distribution or use.

The recall was initiated after the firm received customer complaints of glass in the product. The recalling firm has notified the manufacturer of the findings in order to conduct an investigation as to what caused the problem.

Alimentos Congelados, S.A. Recalls Frozen Broccoli Cuts Because of Possible Health Risk

Alimentos Congelados, S.A. (Pinula) is voluntarily recalling 1,800 cases of Frozen Broccoli Cuts because it has the potential to be contaminated with Listeria Monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

The Frozen Broccoli Cuts were distributed to stores in the following states: Indiana, Kentucky, Ohio, Tennessee, Virginia, West Virginia, Florida, Georgia, Alabama, South Carolina and North Carolina.

The affected Frozen Broccoli Cuts were distributed in poly bags under the following label and code:

  • WYLWOOD Fresh Frozen Broccoli Cuts, NET WT. 16 OZ (1 LB), UPC 5193300110, with bag code: A25335P and A15335P

The company has not received any complaints in relation to this product and is not aware of any illnesses associated with the product to date.

The recall was the result of retail package of Frozen Broccoli Cuts being tested by the State of Ohio Department of Agriculture. The Frozen Broccoli Cuts had tested positive for Listeria Monocytogenes. The company has ceased distribution of Frozen Broccoli Cuts, and is fully cooperating with regulatory agencies.

Consumers who purchased the Frozen Broccoli Cuts are urged not to consume this product and throw it away. Consumers requiring refund or with questions can contact the company at 1-800-888-4646 and ask for Consumer Affairs Monday thru Friday between 8:00AM and 5:00 PM EST.

THOMASVILLE, Ga. – Flowers Foods, Inc. (NYSE: FLO) is voluntarily recalling Cobblestone Bread Co. Wheat English Muffins with the UPC # 0 72250 01316 1 and best by dates of October 28, 2015 through April 10, 2016 because they contain undeclared milk. People who have allergies to dairy products run the risk of serious or life–threatening allergic reaction if they consume these products. No illnesses have been reported to date.

The recalled product involves distribution to retail stores in: Connecticut, Delaware, Kentucky, Maine, Maryland, Massachusetts, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Rhode Island, South Carolina, Tennessee, Vermont, Virginia, Washington DC, and West Virginia.

The voluntary recall was initiated after Flowers discovered that product containing milk was distributed in packaging that did not reveal the presence of milk.

Approximately 10,000 packages of English muffins are involved in the recall. Flowers issued the voluntary recall and is advising its trade customers to withdraw this product from sale. The company is in the process of recovering the product involved and is in contact with the U.S. Food and Drug Administration to ensure the continued safety of those consumers who may be impacted by this issue. The company has also reported the recall to FARE (Food Allergy Research & Education).

Consumers who have purchased Cobblestone Bread Co. Wheat English Muffins with the UPC and dates noted are urged to return to the place of purchase for product replacement or refund. No other Cobblestone Bread Co. brand products are included in this recall.
